York Anticoagulation Warfarin Monitoring Service
Previously the monitoring of anticoagulation for patients taking oral anticoagulants has been preformed in a hospital clinical setting. This activity has now been transferred into primary care.
It has been shown that anticoagulation control by GP Practices, is as good as in hospital and is more convenient for patients. It is quicker and safer and is a more efficient way of carrying out this care.
Our 11 member GP practices provide the Anticoagulation Service in York. This means people on warfarin can be seen locally by fully trained and experienced staff, rather than going to hospital for their blood test.
As an ‘at scale’ provider, we're responsible for the monitoring and management of this service.
We work with patients, carers and other healthcare professionals to improve the safety of these patients, offering specialist advice on any issues relating to anticoagulant therapies.
