Frailty Clinic
What is this service?
This service is provided by YICT and York Hospitals NHS Foundation Trust.
This is a service that looks at all the factors affecting your health & wellbeing, the service supports patients to live well alongside their existing long-term conditions. In this clinic we want to understand what matters to you. We hope to: Minimise the impact of any health problems you may have, support you in maintaining as much independence as possible and work with other health and social care teams to support you in staying well.
The service provides a one-stop clinic, where you will be seen by a Consultant /GP, Physiotherapist, Social Prescriber and Health Care Assistant. We will arrange a home blood test before the appointment and free transport to take you to and from the appointment. Anything discussed on the day will then be followed up at home by one of the York Integrated Community Team nurses, along with any other referrals such as occupational therapy and dietician.

Please note we are currently not taking any referrals at this time due to the clinic being paused.

Not taking any referrals as of 6th March 2024.
