What is this service?
The York Frailty Hub is part of York Integrated Community Team. The service works together as One Team with various services & GPs commissioned to provide care and support to citizens in York that are vulnerable, frail and at increased risk of presenting in crisis. This service aims to find solutions to avoid admissions.
