NHS Health Checks
What is this service?
NHS Health Checks are designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia.
Who is it for?
Adults aged 40 to 74 years living in York who do not have any of the following pre-existing conditions:
- Heart Disease
- Chronic kidney disease
- Diabetes
- High blood pressure (hypertension)
- Atrial fibrillation
- Transient ischemic attack
- Inherited high cholesterol (familial hypercholesterolemia)
- Heart failure
- Peripheral arterial disease
- Stroke
- Currently being prescribed statins to lower cholesterol
As we get older, we have a higher risk of developing one of these conditions, and an NHS Health Check helps find ways to lower this risk.
Where is it based?
How can I access this service?
Patients are invited to book an appointment at these clinics.
NHS Health Checks are available by invitation from a patient’s local GP surgery. Our 11 member practices receive a booking link to invite eligible patients.
Patients can also request an appointment via the NHS Health Check help desk nimbuscare.healthchecks@nhs.net – this is subject to availability and they will be contacted by a member of the Nimbuscare team when an appointment becomes available.
Who provides this service?
Appointments are with a health care professional, usually a Health Care Assistant.
This service is provided in conjunction with City of York Council who also provide the Health Trainer service. This service gives free, confidential one-to-one support and guidance, face-to-face or remotely, on the following:
- Healthy eating advice and weight management
- Finding new ways to be more active
- Quitting smoking
- Reducing your alcohol intake
- Helping you find groups and activities to get back out in the community
How long does the health check take?
This health check takes about 20 to 30 minutes. The health professional will ask questions about the patient's lifestyle and family history, measuring height and weight. They will also take a blood pressure reading.
