Nimbuscare is a not-for-profit organisation that aims to be a collaborative and agile provider that cares for its people and patients. Nimbuscare is based in York and is owned by all the practices in the city, however we deliver services over a wider footprint.

Our Work

  • We deliver Urgent Care services in collaboration with the hospital and GP practices in York, Scarborough, Whitby, Malton and Selby.
  • We provide community-based frailty care, which focuses on treating people in the community where we can.
  • We aim to shift services into the community where possible, for example our community diagnostic centres run in partnership with the Acute Trust.

Care Co-ordinator (Community) – York

Want a role where you can make a real difference every day?

We are looking for 2 x Care Co-Ordinators to join our Community Frailty Team, supporting vulnerable patients by coordinating care, managing referrals, booking appointments, and working closely with clinicians and healthcare partners.

Salary: £25,065–£26,734

Contract: Permanent

Working pattern: 22.5 or 28.5 hours per week

GP - Initial Health Assessments

We are seeking motivated GPs to undertake Initial Health Assessments (IHAs) for Looked After Children (LAC) and Unaccompanied Asylum-Seeking Young People (UASYP).

This is an opportunity to make a meaningful impact by identifying unmet health needs early and supporting improved outcomes for vulnerable children and young people.

Virtual Frailty Ward GP - Scarborough

The post holder will deliver high-quality, patient-centred acute frailty care within the Humber Virtual Frailty Ward, working closely with the Community Frailty Hub to prevent unnecessary hospital admissions and support safe care in patients usual place of residence.

The role supports a no wrong door approach to integrated frailty care focused on recovery, independence, and avoiding unnecessary hospital attendance.

This is predominantly a face-to-face role based at Scarborough, with visits across Scarborough, Whitby, and Ryedale.

Salary: £203.96 to £238.48 a session (dependant on qualifications)

Frailty GP - 1 Year FTC

We are seeking to appoint a number of GPs with an interest in frailty to support the delivery of various innovative Frailty Services as part of the North Yorkshire Community Frailty Hub.

The postholder will undertake a variety of duties, primarily providing clinical advice and support to system providers regarding vulnerable patients in the community as part of a multidisciplinary team.

Frailty Nurse Team Leader

The successful post holder will work as a dynamic and proactive practitioner supervising a team of nurses and health support workers with the support from the clinical lead within the Integrated Neighbourhood Teams (INT) of the City. You will be based in the community, working collaboratively as part of our Frailty Hub MDT supporting the team and conducting proactive nurse led Comprehensive Geriatric Assessments to people living with Frailty in York, to support them to live well for longer in their own homes.

You will also work as part of the Complex Frailty INT in which we work closely with patients with complex needs. The service brings together Primary Care, specialist frailty clinicians, and wider health and wellbeing partners to deliver a coordinated 9-month intervention, focused on stabilising health, reducing risk of crisis, and supporting people to live well in their own homes.

Shortlisting for this role will be undertaken on an ongoing basis, and suitable candidates may be invited to interview prior to the advertised closing date. We therefore encourage early applications, as the vacancy may close sooner if an appropriate candidate is identified.

Salary: £38,880.72 to £41,033.12 a year depending on experience. Pro-rata if part time working.
