Our Plan for Better Local Healthcare (2026–2027)
We want to make healthcare:
- Closer to home
- Easier to access
- Better connected
- Focused on prevention as well as treatment

What this means for you
Over the next year, we will focus on:
Care closer to home
We will support more services to be delivered in local communities, reducing the need to visit hospital where possible.
Better joined-up care
Healthcare professionals will work more closely together so your care feels more coordinated and seamless.
Responsive urgent care
We will continue to improve urgent care services so you can get help quickly when you need it.
Support for people who need it most
We are strengthening care for:
- Older people and those living with frailty
- People nearing the end of life
- Families and carers
Our aim is to provide compassionate, personalised care at the right time and in the right place.

Working with you
Your experiences matter. We will:
- Listen to patient feedback
- Involve patients and carers in improving services
- Work with local communities to shape care
