Frailty assessment service
What is this service?
This is a preventative service aimed at catching patients before they have falls or their condition deteriorates, as well as addressing chronic conditions and identifying any mental, social or physical health and memory concerns early. The aim is to reduce the number of hospital admissions by keeping patients healthy and safe at home for as long as possible.
Who is it for?
Patients who are showing signs of frailty and at risk of deteriorating clinically in the months ahead.
Where is it based?
Clinics take place at Acomb Garth Community Care Centre every Wednesday, from 9am to 1pm.
How do I access this service?
Patients who would benefit from this service should speak with their GP about it.
Who provides this service?
This is a collaborative service involving clinicians from York and Scarborough Teaching Hospitals NHS Foundation Trust, York's GP practices, York Integrated Care Team (YICT) and York Centre for Voluntary Services.
